“The hands are also particularly prone to dryness, which can accentuate signs of aging. Regular skin hydration with application of rich, emollient creams is essential,” note Drs. Antioxidants can support both prevention and rejuvenation, Dr. Mahabir adds. “Vitamin C can brighten pigmentation while defending against environmental damage,” Drs. Alster and Hartman say. “IPL is the go-to for brown spots and hand rejuvenation,” says Dr. Mahabir, noting that it’s safe and effective, albeit a little spicy. When there’s already visible damage, such as brown spots or crepey skin, Dr. Hopkins also turns to IPL treatments. “IPL and other broadband light treatments target pigmentation, while fractional lasers improve both discoloration and texture by stimulating collagen remodeling,” explains Drs. Alster and Hartman.

Dr. Hopkins sometimes employs the Fotona laser, using a three-step approach to boost collagen and refine the epidermal layer. “This is done over the dorsal hands, as well as on the fingers, which can also look crinkled and crepey over time.” She also likes to combine fractional laser resurfacing with topical PRP applied using CuraCator. “This is very similar to the technique I use to treat the face and even the upper and lower eyelids,” she says.

Drs. Alster and Hartman caution that “more aggressive ablative laser resurfacing should be approached with caution on the hands, as there is a higher risk of scarring and dyspigmentation compared to the face.”

Fillers

“One of my favorite procedures to perform in my practice is injections to help restore volume to the thin skin that looks veiny and aged,” says Dr. Hopkins. Drs. Alster and Hartman often opt for biostimulatory fillers, “as they provide immediate volume restoration while stimulating long-term collagen and elastin production, leading to improved skin quality over time.”

Dr. Hopkins points to Radiesse as a strong option, noting that it can be injected using a microcannula to minimize trauma and bruising. “I personally do this for my hands every 18 months,” says Dr. Hopkins. Hyaluronic acid fillers can also be “effective for restoring lost volume, helping to soften the appearance of prominent veins and tendons,” Drs. Alster and Hartman note.

Fat Grafting

When volume loss is more pronounced, Dr. Mahabir prefers fat grafting as a longer-term option to rejuvenate the hands. “Fat grafting in conjunction with other surgical treatments is often a nice adjustment,” says Dr. Mahabir. It’s especially useful when fat grafting is already being performed elsewhere, such as the face or breasts, since the patient’s own fat is already being harvested and prepared.
